2. Fill it out and mail it to our office. Be sure to include the signed "Release and Agreement" form, participant photo, and $100 non-refundable registration fee with your application.
   

PLEASE NOTE THAT MLSA WILL NOT PROCESS YOUR APPLICATION UNTIL WE HAVE RECEIVED YOUR $100.00 DEPOSIT BY PERSONAL CHECK, CASHIER'S CHECK, OR MONEY ORDER. AT THIS TIME, MLSA CANNOT ACCEPT WIRE TRANSFERS, CREDIT CARDS, OR DEBIT CARDS.
